Birth and Death Certificate Information

The Vital Statistics department of the County Clerk’s office has birth and death certificates for Johnson County.  The Clerk’s office is also connected to the Vital Statistics office in Austin, and can obtain birth certificates from anywhere in the State of Texas.  This only applies for birth certificates.  The County Clerk cannot obtain death certificates from anywhere except Johnson County.

Fee

The fee for obtaining a certified copy of a birth certificate is $23.00 each. 
We do not accept personal checks.  We accept cash, Visa or MasterCard.
 

The Fee for obtaining a certified copy of a death certificate is $21.00 for the first copy and $4.00 for each additional copy of the
same record.

Qualifications for Obtaining a Birth or Death Certificate

    In accordance to Section 191.051 of the Health & Safety Code qualified applicants must be:

  

The registrant or the person of record.

   

The parent, child, brother, sister, spouse, grandparent, legal guardian or managing conservator of the registrant.

   

Legal representative, agent or other person acting under contract for the registrant.  Must provide a letter of permission and copy of identification from the registrant.
